Request for Presentation or booth at your event!

Collin County Health Care Services (CCHCS) provides educational outreach presentations and informational booths to improve the health and well-being of Collin County! If you would like our presence at your function, please complete the Presentation Request Form below.

To Schedule a Presentation or Informational Booth

  1. Complete the Collin County Health Care Services Presentation Request Form
  2. Your email and phone number are required so we can contact you regarding your request. We will follow up with you upon completion of the form.
  3. You will be required to provide a time and date of the event and it would be preferred if you would include a secondary date should we be unable to attend the primary date.
  4. We ask that you please select what equipment will be available on-site for the event so we know what other items are needed.
  5. CCHCS will contact you regarding your request and confirm staff availability.

*Please submit at least 30 days in advance. Requests are filled based on staff availability and completing the form does not guarantee we can attend.
